This initial assessment helps us create a personalized support plan that aligns with both medical needs and life circumstances. Whether your loved one is recovering from a hospital stay, managing a chronic illness, or simply in need of extra guidance navigating care options, this evaluation ensures no detail is overlooked. What We Evaluate: Emotional & Mental Health Needs Identify signs of depression, anxiety, grief, or emotional stress and determine if counseling or referrals are needed. Social & Family Dynamics Assess the patient’s support system, caregiving structure, and any family challenges that could impact care. Financial & Community Resources Evaluate eligibility and access to benefits, financial aid, or community services (transportation, food, housing). Long-Term Planning Needs Determine if advance directives, long-term care discussions, or facility placements should be initiated. Safety & Environment Review living conditions to ensure safety and accessibility, and identify areas where improvements or referrals may be needed. Caregiver Support Needs Assess caregiver well-being and discuss possible support options like respite care, training, or support groups.
